Medicare LCD Guidelines

Medicare Local Coverage Determination Guidelines determines what can be covered and what can't, and provides requirements that must be met in order to process payments. iMedCodes provides access to the Medicare LCD Guidelines for all 50 states..

According to the U.S. Department of Health & Human Services, "Medicare coverage is limited to items and services that are reasonable and necessary for the diagnosis or treatment of an illness or injury". This is determined by the Center for Medicare & Medicaid Service (CMS). It dictates what's covered and what's not. iMedCodes provides the requirements that must be met in order to process insurance claims.

Correct Coding Initiative Edits

Medicare developed the National Correct Coding Initiative (NCCI) to promote national correct coding methodologies and to control improper coding leading to inappropriate payment in Part B claims.

CCI policies were created based on the vast coding conventions found in:

  • Paper-based CPT manuals
  • National and local policies
  • Guidelines developed by national societies
  • Analysis of standard medical and surgical practices
  • Reviews of current coding practices

The CMS updates the NCCI annually and these are reflected in iMedCodes.
